Acknowledge your mistake as prominently as the original harm (think newspaper corrections hidden in tiny writing on the ads page = bad).

1) you are showing an understanding of the harms you have caused and committing to change by holding yourself accountable and inviting people to hold you to it
2) you prevent other people from following in your footsteps by making the same mistake
3) you are transparently giving people that couldn't bring themselves to forgive or trust you an opportunity to part ways
